What Is It?

  • A mental health service for adults who are living with a mental illness.
  • The goal is to address quality of life issues to assist the person to live and function independently in his/her community.
  • The service recognizes that individuals diagnosed with an illness have strengths and skills.  With support, they are able to meet the challenges that face them in everyday life.  There are two parts of the program:

1. Individual Community Support:
A staff member of Community Support Service will provide case management services and work with the individual in the community.  The individual identifies his/her needs and goals and the assigned Community Support Staff member provides supportive counselling and assistance in achieving them.  Goals/needs may include:

  • Housing
  • Finances
  • Activities of daily living
  • School or work
  • Health services
  • Relationships
  • Legal

The Community Support Service Staff member works with the individual and community service providers and medical or therapy care givers to ensure needs are addressed.

2. The Drop In:
A supportive, community-based social/leisure/recreational program.  It is open to persons previously or currently involved in Mental Health Services.  The participants meet weekly, and are encouraged to take an active part in all aspects of planning and development of activities.  Participants are not necessarily clients of the (individual) Community Support Service.

Drop In Locations:

Bridgewater: Mondays, 3:00-5:00 p.m.; Conference Room, Dawson Centre, 197 Dufferin St.

Liverpool: Tuesday, 1:00-2:30 p.m.; Zion United Church, Main Street

Expectations The individual recognizes his/her need for the service and is a willing participant.

How to Get Service

The individual can contact the Mental Health Program (527-5228) and ask to speak to someone from Community Support Service to refer him/herself.  Referrals are also accepted from the individual’s Mental Health Service providers, doctors, and community agencies.

Upon receiving the referral, the assigned Community Support Service staff member will meet with the person for an initial interview and needs assessment.
